I hereby provide my consent with the collection, maintenance, and processing of personal data included in this questionnaire by FLY FOR FUN s.r.o pursuant to Section 5 of Act no.101/2000 Coll., on Personal Data Protection.I´m aware of the risks connected with flying course and i proclaim that I undertake it on my own risk.

I hereby confirm that I am not currently attending/planning to attend more than one flight training in the same period. (Student can only do one training for qualification/licence at the same period – for example student can´t do IFR and CPL trainings together – one training must be finished before the other one can start.)

I require the issuance of a certificate of commencement of training at an aviation school in order to obtain a Schengen visa to the Czech Republic for the purpose of studying.

We would be happy to help you with obtaining the visa (determining the required documents and guideline on how to procure them, preparation of the documents and application forms for submission, arranging the appointment with the embassy, after arrival registration etc.). The cost of the service is approx. € 611 (739 with VAT) + related disbursements (translation cost, courier cost, notarization cost etc.). The related disbursements cannot be accurately estimated upfront, usually do not exceed CZK 10.000 and will be re-invoiced to you at cost level.
